MOTS-c is a 16-amino-acid mitochondrial-derived peptide (Met-Arg-Trp-Gln-Glu-Val-Val-Lys-Lys-Gly-Arg-Leu-Ser-Phe-Ser-Gly) encoded within the 12S-rRNA region of mitochondrial DNA. In cell and animal models it acts as a rapid “metabolic stress signal,” translocating to the nucleus and activating AMPK-linked transcription programs that favor glucose uptake, fatty-acid oxidation and antioxidant defense. Recent mechanistic work shows MOTS-c binds and stimulates protein-kinase CK2, a target that helps explain its ability to preserve skeletal-muscle insulin sensitivity and endurance capacity in rodents subjected to metabolic stress. The first human analogue, CB4211, completed Phase 1a/1b testing in adults with obesity and NAFLD, where four weeks of daily sub-cutaneous dosing was well-tolerated and produced double-digit relative reductions in liver fat versus placebo.
